Aging at Home Strategy continues to improve seniors’ lives

A further $1.6 million will be put to work in matching the needs of seniors and their caregivers with local supports to help them remain healthier in their own homes. The funds are part of remaining unallocated second year Aging at Home funds and were approved by the South West LHIN Board of Directors at its December meeting.

Implementation of our Integrated Health Service Plan

With a 12-year Blueprint for the future of the health system of the South West LHIN as a guide, implementation planning for the next Integrated Health Services Plan - 2010-2013 is well underway.

Achieving our blueprint vision cannot be done in isolation. We are actively engaging our health service provider boards to reaffirm our system priorities, and to ensure that provider priorities are in alignment with both our blueprint and IHSP. By viewing the future of health care from a system perspective we will be able to meet the needs of individuals and communities through an integrated continuum of services.

Flo Collaborative – Navigating the system

In September 2007, the Change Foundation’s Centre for Healthcare Quality Improvement (CHQI) launched a province-wide initiative intended to help Ontario’s healthcare system provide the care required by a representative patient named Flo and thousands of others like her. Called the “Flo Collaborative,” the aim was to address processes of care delivery — to eliminate the hassles, bottlenecks and irritations — so that Flo’s transitions from acute hospitals to other settings were faster and smoother for everyone, including Flo, her family and the staff who care for her.

Over 600 long-term care beds opening soon!

Over the next three months, more than 350 new long-term care (LTC) home spaces will become available in London as two new homes open their doors to residents. By mid-2011, two additional new homes will open, adding another 250 spaces.
