As a member of the South West LHIN Health Links Team, the Quality Improvement Coach is responsible for enabling, supporting and sustaining improvement efforts within the Health Links strategic and work plan. Areas of responsibility will include facilitating improvement initiatives such as the Health Links Learning Collaborative, transfer of learning, on-site coaching and performance monitoring. The Quality Improvement Coach leads, participates in and demonstrates an understanding of quality risk and patient safety principles and practices. Follows all safe practices and procedures to support a safe patient and working environment.
