Reporting to the Vice President, Strategy, System Design & Integration, and working closely with the Sub-Region Clinical Lead and the Director Home and Community Care, the Director, Planning & Integration provides leadership for the engagement, planning, integration, and improvement initiatives at the sub-region level. Regional Palliative Care Network Lead

As the Regional Palliative Care Network (RPCN) Lead, this position will lead work with partners to develop and implement an integrated Hospice Palliative Care System that spans the continuum of care across the South West. The Lead will drive improvement in the delivery of Hospice Palliative Care within the South West and facilitate collaboration amongst partners to enable the establishment of an integrated Hospice Palliative Care System based on best practice care and in alignment with provincial direction for Hospice Palliative Care in Ontario as driven by the Ontario Palliative Care Network (OPCN).

Regional Palliative Clinical Co-Lead (Non-physician) 

The Regional Palliative Clinical Co-Lead will collaborate with local clinicians and other partners to improve hospice palliative care across all patient populations, illness trajectories and health care settings. Together, with the South West Hospice Palliative Care Network (SWHPCN) Lead, they will be responsible for the delivery of regional work plans and completion of annual objectives according to the goals of the Ontario Palliative Care Network (OPCN), and implementation of the Health Quality Ontario and Ontario Palliative Care Network quality standards. Telehomecare Clinician

Telehomecare is a patient-centered approach in the delivery of efficient and effective care to people with chronic disease.  Leveraging information and communications technology (ICT) to remotely link providers and clients, Telehomecare provides a targeted and rapid response from the care team to an emerging health crisis, as well as proactive patient coaching, education and coordination of care amongst multiple care providers. The Telehomecare Clinician provides remote patient monitoring, health coaching, and disease-specific education to patients with complex chronic diseases.  It is based on a collaborative model of care that focuses on enabling the patient to better manage his/her chronic disease through the support of clinician monitoring and coaching.  The Telehomecare Clinician works in collaboration with the patients’ circle of care.
