End User Support Technician

As a member of the Corporate Services Team, the End User Support Technician is responsible for addressing Help Desk concerns and providing front line technology support across the South West LHIN.

Care Coordinators, London (Temporary full-time)

Two temporary full-time positions (approximately 10 months and 12 months) are available in the London location with an initial assignment in the In Office Team. Possessing the skills, the knowledge and credentials (Registered Nurse, Registered Occupational Therapist, Physical Therapist or Master of Social Work), as well as experience and sound judgment, the Care Coordinator contributes to the success of patient-driven care throughout South Western Ontario.
As a Care Coordinator, you’ll leverage your nursing expertise and knowledge of community resources to assess patient needs, determine their eligibility for services and subsequently develop, evaluate and/or revise plans of service for patients. Recognized as a valued member of the Patient Care Team, you’ll be accountable for coordinating the delivery of care to patients across a continuum of care, facilitating and ensuring the achievement of quality clinical outcomes.

Manager, Quality Improvement

Reporting to the Director, Quality, the Manager, Quality Improvement, is responsible for the implementation, and evaluation of a system-wide and organizational Quality Improvement efforts in alignment with vision, mission and values, strategic directions and operational plans of the organization. This approach extends to the measurement, management and quality improvement in partnership with health service providers and the organization’s contracted service provider organizations. Leadership in key areas of system and service quality improvement and management, enterprise risk management, patient safety, ethical decision making, patient relations, and emergency management will be essential in achieving population health, experience of care and value for money. Sub-Region Lead

Reporting to the Vice President, Strategy, System Design & Integration, and working closely with the Sub-Region Clinical Lead, the Sub-Region Lead provides leadership for the engagement, planning, integration, and improvement initiatives at the sub-region level. In addition, the Sub-Region Lead will have specific portfolio and project leadership responsibilities to advance complex planning projects, integration and health system improvement activities aligned with health system priorities.
