Regional Clinical Co-Lead (Physician)

The South West Hospice Palliative Care Network (SWHPCN) in partnership with the South West LHIN and the South West Regional Cancer Program are seeking a Regional Clinical Co- Lead, to join the SWHPCN leadership team. 

The Regional Clinical Co-Lead will report to the Cancer Care Ontario Regional VP and the LHIN CEO (through the Chief Clinical Lead). The Regional Clinical Co-Lead will collaborate with local clinicians and other partners to improve hospice palliative care across all patient populations, illness trajectories and health care settings. Together, with the South West Hospice Palliative Care Network (SWHPCN) Lead, they will be responsible for the delivery of regional work plans and completion of annual objectives.

Director, Communications

As a member of the Senior Leadership Team, reporting dually to the Chief Executive Officer (CEO) and the VicePresident, System Design and Integration, the Director of Communications is responsible for overseeing the provision ofstrategic and tactical communications and media relations including planning, development, implementation andevaluation; providing direction and tactical support to corporate engagement activities; ensuring the overall quality,consistency and effectiveness of all communications activities; directly participating in the development of complex and/orconfidential communication materials on behalf of the CEO; providing information in support of the CEO’s accountability tothe Board; and contributing to LHIN-wide collaborations and initiatives.

Manager, Occupational Health and Wellness

As a member of the human resources and organizational development team, the Manager, Occupational Health & Wellness leads the occupational health, safety and wellness program along with their team of disability management, attendance management, and benefit administration experts for all South West LHIN employees. This role, in conjunction with the Patient Care team, oversees the Infection Protection and Control (IPAC) requirements for the organization. This position will also review and arrange accommodations under the Human Rights Code in collaboration with the Coordinator, Occupational Health and HR Business Partners. This position leads, participates in, and demonstrates an understanding of quality, risk and patient safety principles. Follows all safe practices and procedures to support a safe patient and employee working environment.

Clinical Lead, Regional Wound Care Program (Temporary full-time until November 2019 with the possibility of extension)

The South West Regional Wound Care Program (SWRWCP) advocates for the integrated seamless, timely and equitable delivery of high quality, safe, efficient, and cost effective, person-centered, evidence-informed skin and wound care to the people of the South West Local Health Integration Network (LHIN), regardless of the setting. The SWRWCP is hosted by the South West LHIN but supports all hospitals, long term care (LTC) homes and contracted community nursing service provider agencies. The SWRWCP advocates for the South West health system partners to become leaders in integrated skin and wound care management. Care Coordinator

 

Possessing the skills, the knowledge and credentials (Registered Nurse, Registered Occupational Therapist, Physical Therapist), as well as experience and sound judgment, the Care Coordinator contributes to the success of patient-driven care throughout South Western Ontario.
As a Care Coordinator, you’ll leverage your expertise and knowledge of community resources to assess patient needs, determine their eligibility for services and subsequently develop, evaluate and/or revise plans of service for patients. Recognized as a valued member of the Home and Community Care Team, you’ll be accountable for coordinating the delivery of care to patients across a continuum of care, facilitating and ensuring the achievement of quality clinical outcomes.

Director, Home and Community Care: Mental Health

Reporting to the Vice President, Home and Community Care, the Director is responsible for providing leadership and direction for the coordination and delivery of home and community care. This position oversees a team of managers, care coordinators, and patient care assistants who support home and community care patients, in collaboration and partnership with various health system sectors and services, in the South West LHIN. An area of focus for this role will be leadership and experience in the area of Mental Health, including working knowledge of, and system design within, the Mental Health sector.
As a strong healthcare leader, you are required to have a high level of independence, provide leadership in the delivery of excellent care coordination and to establish strong collaborative partnerships with community services partners, hospitals,
contract service providers, and other health system planners.

Health System Planner

Reporting to the Sub-Region Lead for Huron Perth, the Health System Planner is responsible for supporting effective planning, coordination and implementation of system design and integration priorities across the local healthcare system and within a sub-region, to improve patient experience, health outcomes and value for money. This position is responsible for providing technical planning expertise in the analysis of data and information that assists in determining LHIN-wide and sub-region plans and priorities, including integration opportunities. The Health System Planner provides technical expertise, advice and support in the development and management of work plans and reporting processes for LHIN sectors and sub region portfolios.

Manager, Planning and Integration

Reporting to the Director, System Design, Integration & Digital Health the Manager, Planning & Integration is responsible for supporting the Director in the leadership of planning and integration strategies to achieve improved patient experience, health outcomes and value for money. The Manager, Planning & Integration supports the development and alignment of system-wide and sub-region system design and integration plans and initiatives to advance the mandate and long range objectives of the LHIN. The Manager ensures common practices and policies are in place related to planning and integration and is accountable for ensuring alignment across the LHIN in conjunction with Manager counterparts within the other functional areas. In addition, the Manager is responsible for the development, coordination and tactical oversight of team members based on organizational priorities, including the planning, implementing and monitoring of large and small initiatives, stakeholder engagement, and coordination of program performance targets.

Practice Facilitator: Health Links

The Improving Access to Team Based Care Facilitator will be a champion of and support all change and improvement processes that enable adoption of improving access to team-based care practices and operations. A focus of the facilitator’s role will be to engage all implementation partners in creating an environment of respect, appreciation and open-mindedness, for collaborative decision-making and co-learning in patient care. Facilitators will work to connect non-team primary care providers (primarily physicians) and their patients with complex care needs, to inter-professional care teams and play a key role in helping non-team providers and care teams work together to develop common goals, effective communication pathways and robust, trusting working relationships. Facilitators will also work with the provincial research and evaluation team to collect and analyze data and they will be responsible for reporting back to non-team providers and care teams and supporting their active engagement in continued practice-based learning, and program improvement. Additionally, facilitators will work with non-team providers and care teams to build local communities of practice and identify learning initiatives that will support sustainability of shared cared and promote continued collaboration, integration, and quality improvement in primary care.
