The South West LHIN is seeking a dynamic physician to work as part of a team to help achieve the goals and objectives of the South West LHIN’s Integrated Health Service Plan - focusing on the Strategic Direction to increase access to primary care and contributing to the development of a patient focused integrated system of care for all.
The Primary Care Co-Lead will provide leadership to strengthen and advance primary care within the South West LHIN through enhanced collaboration and integration across the health care system. Working with other physician leads, primary care physicians and other health professionals, LHIN staff and health system partners, the Primary Care Co-Lead will promote a team approach to health care aimed at improving health outcomes, experience of care and value for money within our health care system in the identified geographic area.
