Home and Community Care
Ontario is committed to developing a health care system that puts patients first. Anchored in the province’s Patients First: A Roadmap to Strengthen Home and Community Care, the 14 LHINs across the province have been actively planning for these changes. In our region, the current work of implementing the policies related to the Expansion of Personal Support Services (PSS) and the Community Support Services (CSS) Integration are now being planned taking into account proposed fundamental shifts in health care.
The creation of one home and community care sector will be one of the cornerstones of health care transformation. This will mean creating a home and community care model that will allow people to experience services as seamless, timely, coordinated and easy-to-navigate-and-use.
Below, you will find links to the Patients First documents, the South West LHIN’s Integrated Health Service Plan for 2016-2019, Guideline documents CSS & CCAC, and webcast materials. All of these materials are recommended readings for agencies to support future conversations and planning.
