The South West LHIN is leading the way in transforming the health system to better meet the needs of our communities and to ensure a healthier tomorrow. Transformation means collaborating with health service providers – long-term care homes, community support services and hospitals – in redesigning effective approaches in the delivery of health services and to integrate services for more seamless care. Transformation also means setting performance goals and ensuring accountability in meeting these goals.
To guide this transformation we have a three-year strategic plan, the Integrated Health Service Plan (IHSP) 2013-2016. The IHSP outlines numerous programs and initiatives to help people live healthy, independently and safely at home. Click here for more information on the IHSP.
